All right. So week one, the NFL going to get to some of those injuries.

4:03We'll get to some stuff in college. I text you. I want to talk about meniscus. Because so Brock Bowers for the Vegas Raiders, like he's got a meniscus and. I think that is an injury that can come with so many different variations. And he's getting a trim and he's supposed to be back in two weeks. I mean, Matt, I've talked about it before. I got a trim eight days before the sugar bowl and playing the game. And there's some that you get repaired and you're not playing in eight days, right? If they have to totally repair it. Like how many different variations of a meniscus injury can there be? And because if correct me if I'm wrong, you don't grow more correct? Correct. And so like once it's gone, it's gone. Once it's gone is gone. Yeah. Okay. You don't grow more and outside of like a really exceedingly small percentage of these, they don't heal either. So once it's torn, it's torn. There's a couple of cases that you tear it in the perfect location. Somebody who's young, they might get some some recovery of that. But typically we see a meniscus tear. And it's going to be and it doesn't grow back.

5:05So you have to decide first and foremost, is it going to be where you're just going to shave it down? Or are you going to go in and try and actually repair the meniscus? And some of that depends. There's a lot of variables where where the meniscus is the tear. So the meniscus is it's a little cup of cartilage. You have one on the inside, one on the outside. So you have a medial and lateral meniscus. And as you get further away from where it attaches around the periphery and get more towards the central portion of it, it becomes less and less vascular. So it has less blood flow. And so those are tears when it's really further from the attachment sides, further from the good circulation. Those are typically ones you're probably going to shave out. You know, it's not always quite that simple. There's always a lot of variability. It can be quite complex. But typically you're not going to try and fix one if it's an area that has a bad blood supply for the most part. There's some newer procedures and techniques that might, you know, might allow you to do that on occasion. And then some of it looks. Sometimes you talk to the individual and you know, look, you got a guy like, you know, Brock Bowers. And you know, he wants to be on that field. He doesn't want you missing time. And because if you had to repair your out for, for usually a few months, you know, that's a longer recovery.

6:06You know, if you get that repair versus the menacectomy where you're shaving it down, well, those guys, yeah, at your point, you know, eight days, a few weeks, as long as there's not a lot else going on. And that's not, that's not unreasonable. Now I did after my rookie year, I came back to Baton Rouge. And because I had always, I had like, I think two surgeries before from meniscus at LSU. So just always something that was chronic for me. And I came back after my rookie year. And I remember they said, hey, like whatever you do, don't micro fracture. And then I went under and they were like, we had to. Like we just, it's something we had to do because I remember waking up and the first thing. You're like, well, we had a micro fracture. I was like, wait, hold up. They said, whatever you do, don't do that. Why do you have to do that? And what is that procedure? And what does that do? So micro fracture is actually not for the meniscus, but it's for the cartilage. So, okay, you have, you have, if you think about the knee, you have two bones that come together, the femur and the tibia. Each of those bones has a layer of cartilage that's on the bone. And that's kind of what we call, what we call, conjural cartilage.

7:06So that's cartilage that's basically right on top of the bone. And then the meniscus is sort of, so that's almost like the bread of the sandwich. Yeah. And then the meniscus is sort of the meat in the sandwich, in the between. And so if you have damage to the cartilage that's on the bone, the conjural cartilage, then there are different procedures. That's not a meniscus repair. And a lot of times the two will happen together. A lot of times you'll see some. And I think we had talked about this several weeks back. You know, what are some of those injuries that are the most difficult to know what to do with? Cartilage injuries are hard. Yeah. Cartilage injuries, you got micro fractures, you've got other procedures where you can put a little, you know, aloe graph to bring a little, a little, you know, segment of cartilage in and trying to get it to grow. You can do all kinds of things, but none of the treatments are great. It took me a while to come back. And it's a longer recovery. So, so why is it, why is it tough? It's a less blood flow to the cartilage. So that what you're trying to do with micro fracture, you're more, actually, you're literally drilling holes into the bone. Yeah. So, so micro fracturing, you're drilling small little holes, trying to stimulate some bleeding from the bone, even some of the bone marrow potentially. And the hope is that that will stimulate the cartilage that you have to maybe get a little healthier.

8:07Right. Maybe you'll get a little bit of growth of more of a highland cartilage, which isn't quite as robust and thick. So it doesn't always work as well. But, you know, it does, and it is a longer recovery. Every time you add a complexity, you're adding time. Yeah. So, go further into the highland cartilage. Highland. So, you have contract cartilage, highland cartilage. When you have a perfectly healthy joint, you have this really thick, kind of, you know, cushiony cartilage. It looks great. It's very beautiful. Yeah, it's beautiful. The highland cartilage is more, almost like scarring. So, it's better than nothing, probably, but it's not as good as what you were, what God gave you. And that's a lot of times what we're stimulating with a lot of the treatments that we do. You know, sometimes that's the best you can get. Right. And so... So, there was a week one kind of injury, but a lingering mild scare. I mean, probably the biggest trade in the history of professional sports, or one of the biggest trades in history of professional sports, mild scare to the Rams. He is in place on IR right now for a handful of weeks, and he is going to have arthroscopic knee surgery.

9:10And now, the injury isn't real specific about what is actually hurt in his knee, but there was fluid, some swelling, and some pain. Like, what... Can you, like, narrow down what it could be? Arthoscopic surgery is like, we'll use for these a couple of different things. Yeah, I think there's things you know that it definitely isn't. You know, when you're talking about surgery, if it's a big ligament injury, you know, an ACL or something like that, you know that's going to be a much longer recovery. If it's a meniscus injury, you know, could he have had some type of meniscus pathology? Maybe his a little bit more in depth than the one that Brock Bowers had, maybe because it was more chronic, maybe it's going to take a little more clean out, maybe there's more irritation. So, not all meniscus surgeries when you're doing a menisectomy are, you know, that one to two to three week variety. A lot depends on what else is going on. Could there be a little bit of cartilage that's just kind of flapping around, free bodies, other things? So, it's probably something in the meniscus sort of cartilage, you know, kind of clean out type realm. But not knowing more, it's hard to know exactly. So, a scope is arthroscopic surgery, right?

10:10Right. And I know you've kind of talked about the details of that, but you hear about like guys, you know, getting something shaved down or something like that. Is it like literally, I think about like shaving a callus on your foot down. Is it literally kind of like they take a- Pretty similar. Yeah, pretty similar. Yeah, so you know- It's simple as that. And again, obviously it's going to be a little more complex for sure. But for the guys that do a lot of these surgeries, you know, they understand that knee anatomy. So for them, it probably isn't much more complex than shaving down a callus. When you look at somebody who does this every day, day in and day out, but you have to be mindful of where you're going. You don't want to shave too much down because again to Jason, one of your shaved, the meniscus doesn't come back. So if we shave too much of your meniscus now we're leaving with less meniscus. And look, you're designed to have a meniscus for a reason. Right. So the more of it we take out, the more problems you may have down the road. You might feel great right now, but you have that much less support. So again, with everything there's a little given take. So it's not quite as simple, but yeah, from the procedure itself, it's not that different. Hi, this is Dave Roberts.

Doc, one of the things I want to ask you about now, no, we don't talk specific injuries, obviously with LSU, but injury reports. Those are now new in college football and they have to be released. What is the time they have to be released? And I guess maybe what are some of the guidelines that have to be followed? Is it a guy didn't practice or if a guy has anything? He comes into the training room for any kind of treatment he has to be put on there because I believe that's more of the NFL model.

14:12If you, you know, if you're getting treatment on something that has to disclose it. Yeah, and so the injury reports go through the out of the trainer. So I might be a little bit inaccurate, full of disclosure because this is new for us. We help with it, but they are the ones that present it. Typically it is, yes, what is the likelihood they're going to play? That's really a guy who's missing practice, a guy who might not play. Right. So I have to put them on there. There is a requirement. It does follow a similar model to the NFL where it's probable questionable, doubtful and out of it. So I mean, if a guy has got, I think he's got less than a 20% chance or something like that, I'm missing. Then he's probably probable. If it's 50% he's doubtful or greater than 50% it's doubtful. So I mean, there's some variables there. We do have to get them in early in the week. And I believe the last one comes in the day, but you have to kind of update it a couple of times throughout the week. And it's definitely not as regimented and fine-tuned as the NFL model. Right. But they want to know. They want that information. And if they find out that, hey, we had a guy that wasn't practicing all week, we'll get in trouble. Yeah.

15:12We can get some fines for that or penalties for that. Let me kick you out of the SEC. Or kick us out of the SEC. Sure, sure. That's a thing. Apparently. We love to threaten this. So yeah, that has changed a lot of the complex. To be honest, we're still kind of learning this. This is only a few years in. And so I said, I don't, I'm probably, I said, I don't want to butcher it too much, but it is a new thing. And I know the trainers, because they've already got enough on their plates too. Yeah. You know, they're, they're, they're working really hard just to do their jobs. And then you give them another task. So, so we're trying to figure out how to make that work for everybody. Dr. Gatay, there's a really good question in the chat that I'm really interested in, because I don't know the rules here. Now, I know you'll do a lot with injections, the stem cells, the PRP, bio, what a kind of... So some, yeah, they go on biologics, we're getting them at us. And there's a few different terminologies. So that's all meant to put good cells back into you, stimulate cellular growth. Like, but the question I heard are, there's, are peptides legal for the college level for the athletes to take to stimulate healing and help the healing process?

16:13So, so peptides are becoming a big topic. They're, they're becoming very widely used. And there are a lot of organizations that are starting to ban. They are banned from, from, I think I believe the NFL is a busy ban them. Yeah. And collegiate sports, there are certain peptides that are banned. And, and a lot of professional leagues, they are banned. And if they aren't banned, they're, they're probably heading that direction. So we have to be really careful with recommendations we make. Now, the testing for them is also not great. This is not me condoning, take it, take them because of this. Because there are some ways they can test for some of them. But, so right now it's not, it's not like when we get a, you know, with the NCAA, they'll say, hey, we got these guys behind them, dry test. Peptides are not something they're typically testing for. I think they'd have to have a motivation to do so because it is not a standard test. So it's still sort of an unknown. I typically tell my guys you probably should stay away from them not because I think they're dangerous, not because I think they don't work. But the rules are not 100% clear. And they're just the governing body of that sport. Yeah.

17:13Because I think they're usually if they are banned and so that's because they work. Well, that's right. Look at the one. Typically, that's why I tell people at the very minimum, you know, there's a peptides, there's a big debate out there. At the very minimum, there are people out there who believe they give a benefit or else they wouldn't be on that list. So there are people in high positions whose job it is to know what's out there who are either wrong in that assumption or truly believe that they are. And look, I do think there's something to the peptide market. There's too many people that have had good success with it. But to be fair, there's a lot we still don't know. So it's a little bit gray. But look, I tell patients all the time, you know, you got to get kind of a. If I had an injury, I'd probably take them, you know, but at the same time, I don't really tell my patients to do so because we just don't know all the details yet. All right. I'm going to make sure. That does bring a good question. And you might, I know you're not in all the all the meetings and everything throughout the week. You only have certain days and hours. But like, I wonder if there's been extra communication to the team about. I don't know. Security or just extra protocols for this game that they might not get for other games. Yeah, I think there has been, you know, I've kind of. Been watching and I think they've been talking to me a little bit. I think everybody's just aware like hey, this should. Yeah, this is going to be a different game. You know, look, we played at Alabama in 2019 when when. Trump was there. We all talked about it beforehand that there's going to be a lot more security coming in. Because there was it was more getting in the stadium. We had like secret service at the hotel checking our bags. Once we got on the bus, we could not get off the bus or go get our bag. If we did, we had to go back through the security lines. It was a different protocol than what we normally went through. I don't think it'd be quite that extreme.

19:15But we're anticipating a little bit more security at this one. It's going to be something. Absolutely national guard. Yeah. Dr. Steven that's what you're good enough to join us here. 22:16I break. I'm not not. To pivot quickly to the New Orleans Saints, they got another tough one there, but they got to go to Baltimore. I'm not going to go to the New Orleans. I'm not going to go to the New Orleans. I'm not going to go to the New Orleans. I'm not going to go to the New Orleans. I got to go to Baltimore, take on the Ravens. We talked about this, whenever the schedule was released, you knew it was going to be a tough first two games, certainly for the New Orleans Saints. Again, I had no issue with them going for two. There's so many people that caught into the NFL show yesterday that were like, yeah, but if you look at the history, if you have a tie in your division rankings, and it oftentimes ends up where you make the postseason because of that tie and they cited Green Bay a year ago. And some other teams for sure, but I'm like, yeah, loser mentality. First week of the season. It also doesn't matter. Everybody in your division lost this weekend. Every team.

23:17Right. And that was another one of their points. You'd be up in the division. Okay. You were not guaranteed to tie that game. Right. 90% of the chance they were. It was a mid-a half left. Was it great kicker? And they just went through you like a hot knife through butter. You weren't stopping them. They weren't going to stop you either. Could you imagine if you go for the tie and you lose? Then you lose. Yeah. No. I got no issue. I don't like that. No issue. But. I didn't really heard anybody that has an issue with it. I know the national people probably have talked about. Maybe. I don't think anybody around here. No, I don't think so either. You should because again, like, I think everybody lose in your division means you should go for it. Because at worst you're tied in your division. Yeah. At best you're a game up on everybody already. If you're fine. If Kellinmore kicks it and they go down the field and kick a 60 yarder to win it. Which Jake Bates can do. Kellinmore is getting drug this week here for sure. Yes. My guy, he is getting absolute like,

24:19oh, did the right thing. But now you got to go to Baltimore and. It's been a regular. And it's been a regular. It's been a regular. Record be damn. Oh and one one and oh, oh and one. It's still the Baltimore Ravens. Lamar Jackson looked really good. I don't know how good the Colts are defensively. I think they've got some issues for sure. But Baltimore went on the road and that game wasn't really close. So you got some challenges. And you got some challenges in Lamar Jackson. But you've really got some challenges in Derek Henry. My guy. You struggle with Gibbs this week. And you're worried now. It's different. They're different running backs. I get that. But Derek Henry last week just shows you that he doesn't age. And I don't know if y'all know like his regiment in the all season. Everything he does is to get ready for the season the way he eats the way he trained. The way he sleeps. Like he broke it down for us in August. I mean, it is Matt. It is as tight to like his schedule.

25:21Like it's as tight to perfect sleep, perfect balance and meals. Workout times recovery. Like, you know, a lot of times we'll have stuff like that. Well, you know, 80% follow it or 85% if we're doing really well. He is like 99.999999% following his regiment. And you see it. And that's why I like he's over 30. No, you don't. He's over 30 and he can still be who he is and still thrive. And most of the time 30 is a graveyard for running backs. Yeah, I mean, he's got to be doing that. I mean, that size too for what he's able to do. It makes it even more impressive. Like for whatever reason, like you think of people that make it a long time like that with their body. Maybe a little bit shorter, maybe a little bit more compact. And he is the absolute opposite of that. So what he's able to do and you know, I'll get him in fantasy too. It helped. I like Kenneth Walker helped you. He.

26:21Jesus. Sorry about it. I told you, because Walker's going to go off. But what Derek Henry, he has these games every single season. He's going to have a handful of games where he's going to score three touchdowns. It's just his game, right? And then all of a sudden he's going to have these games where they got a good scheme or they play some really good linebackers or get like they just that it's not it's going to have to be a little bit more Lamar Jackson. And but the combination of Lamar Jackson, Derek Henry, Zae Flowers, plan, like it's been impressive in Lamar Jackson. And you were, he's got he's. It seems like Vegas knew what was going to happen. What they went seven games last year. And that what what that went. Yeah, and they were basically favored in like every game, but like two this year. Yeah, they had I think the for a while, the. The best. The highest over under season wind total. I think it was like at 12 and a half until the Rams traded for miles. Garrett, then they became the favorite by last in their division, by the way.

27:22Yeah, seven wins to the Vegas betting highest winning over under season wind total until the Rams traded for miles. So they know something they knew something was going to happen. Lamar goes 17 to 25, 324 yards in a touchdown at seven Russian attempt 40 yards in a touchdown. And then you put that on top of what Derek Henry did. I was Zae Flowers did in the first half before he went out with a hammy. Yes, he is he out questionable questionable. He did pretty good. He is question is pretty dangerous. Yeah, so you just got to hope that Lamar has one of those games where Lamar looks bad. And he has those games. Contain Derek Henry. And then Tyler Shuck threw for 400 yards in both halves this time. That'd be an awful record. Oh, it like I don't I don't worry about the Saints being able to move the football through the air in this game. But I do worry about if you can't stop Derek Henry.

28:23How many possessions are you truly going to get? Yeah, like that's the thing that you worry about when you face a team like this where. The run-in back can keep the clock going and they can keep possession and they can win the clock in the time of possession battle. But also like on third and seventh when you've got it bottled up and then here goes Lamar around the corner for nine. That breaks your back and pisses you off more than anything. But if you only have two losses and you're heading to that game, I think you have done exactly what you need to do on your schedule. But Ole Miss is pushing everything they have into this game. It feels like, and if they don't find a way to win this game, what that looks like for the next couple of weeks after that? Yeah.
