This episode is the one where Dean's greatest wish has come true, and even though it hasn't come true really, there's enough reality in it for him to be very tempted to stay. Meanwhile, I'm assuming Sam is losing his mind because he's so worried, but we only see wish!Sam who is a very different person in a lot of ways.
