Welcome to Onko Life Center podcast. Glad to be here for this one. You know, what if I told you there is a common cancer that is almost entirely preventable, but we are still relying on a screening schedule from decades ago to fight it. Yeah, it's a wild situation when you really look at it. It really is. I mean, usually when we talk about a cancer diagnosis, the entire conversation just revolves around reaction. Oh, for sure. It's almost always about finding a lump or noticing a disruptive symptom. Yeah, or scanning for a tumor that has already established a foothold in the body. But today, we are flipping that script entirely. We really are. Because the mission of this deep dive is exploring the preventative super power of modern medicine against one very specific disease, and that is cervical cancer. Which is, frankly, a truly unique situation in the medical landscape. So, well, cervical cancer stands apart from most other major cancers, because we actually possess the biological tools to catch it long before it ever develops into a malignancy.

That is a huge distinction. Yes, it is. We are not just looking for early stage cancer today. We're looking for the microscopic conditions that precede the cancer by years, or, you know, even decades. Because when we look at the data on cervical screening tests, we see something incredibly rare in oncology. We see a massive window of opportunity. A huge window, yeah. So, this deep dive is designed specifically for you, the listener, to break down your options. We are going to decode the latest medical guidelines so you know exactly what your body needs, why it needs it, and when. Basically, we're cutting through the clinical jargon to outline a very precise road map for prevention. Exactly. So, to understand how we prevent this cancer, we first have to understand the specific timeline of how it starts, which is actually kind of unusual. It is. So, to start, almost all cervical cancer cases are caused by the human papilloma virus, which is commonly known as HPV. But wait, isn't HPV practically everywhere? It really is. Because our source material notes that most sexually active people will encounter this virus at some point in their lives.

That's true. I mean, it is often described as the common cold of sexually transmitted infections. So, if it's that ubiquitous, why isn't cervical cancer infinitely more common? Encountering a virus and actually developing cancer have to be too vastly different biological events. What's fascinating here is that a high-risk HPV type only leads to cervical cancer if it manages to persist for years. Oh, so it takes a long time. Yeah. When most people encounter HPV, their immune system immediately recognizes the viral proteins. It deploys white blood cells and just eliminates the infected cells naturally. So, the virus is cleared without you ever knowing it was there? Exactly. And without causing any permanent harm to the body, the danger only really arises during a very specific window. Which is when? When a high-risk screen of the virus manages to evade the immune system. It effectively hides out and sets up a long-term, completely silent residents in the cervical cells. Okay, so we are dealing with a slow-moving stealthy threat. The timeline from infection to cellular mutation is measured in years.

Decades sometime. And that completely redefines the strategy for dealing with it. Because if you are waiting for physical symptoms, things like pelvic pain or unusual bleeding, you are already too late. Waiting for symptoms completely defeats the purpose of screening. Precisely. By the time physical symptoms appear, the virus has already won that biological battle. The damage is done. Yeah, the cellular changes have organized into a full-blown malignancy and have begun invading the surrounding tissues. Yeah, be they do it very differently. We are basically dealing with two primary tests today. There's the traditional PAP smear, which has been around for decades,

and the newer HPV test. And to visualize the core difference between these two tools, think about a home security system. I like that. That is a highly effective way to look at the differing methodologies between cytology, which is the PAP smear, and molecular biology, which is the HPV test. So, imagine your cervix is a house. The PAP smear is essentially like checking your home's windows to see if someone has already started prying the moped in. Looking for the physical damage. Yeah, under a microscope, medical technicians are looking for early abnormal cell changes. They are checking the physical damage to the window frame. Now, the HPV test, on the other hand, is like a high-tech perimeter camera spotting a burglar on your property line before they even touch the house. That's a great way to picture it. It is looking for the virus's DNA before those physical cell changes even begin to happen. Yeah, because the PAP smear detects the downstream consequence of the virus. A technician literally stains the cells on a glass slide and looks for enlarged or mis-shape

in nuclei. Which means the virus has already started doing its damage. Exactly. But the HPV test detects the microscopic presence of the virus itself by amplifying its genetic code. The perimeter camera spots the threat before the break-and-even starts. Yeah, it's a completely different level of detection. Okay, let's unpack this. If the HPV test catches the risk much earlier in the process, I mean, if it spots the burglar on the lawn before the window is even touched, why wouldn't everyone, regardless of age, just get the HPV test? It seems like the obvious choice, doesn't it? Yeah, it sounds like the vastly superior security camera. Why wouldn't my 20-something sister just demand an HPV test to be safe? Well, it comes back to the biological timeline and the immune system behavior we discussed earlier. Oh, about clearing the virus naturally. Yeah. Remember that most sexually active people encounter HPV, and most of those infections clear on their own. And this aggressive immune response is especially robust in younger populations. Okay. So if we gave a highly sensitive HPV test to a healthy 25-year-old,

it would very likely come back positive. Wait, really? Just positive by default almost? Right. For a lot of young people, yes. But all that tells us is that she recently encountered the virus. It's like passing someone with a cold on the street. Her immune system is highly likely to clear it naturally within a year or two. Ah, I see. So the perimeter camera would basically be going off constantly for people just walking past the property on the sidewalk rather than actual burglars trying to break in? That is spot on. Testing for the virus at that younger age would cause immense, entirely unnecessary psychological anxiety. Oh, I can imagine getting a positive result would be terrifying. And worse, it could lead to invasive medical interventions, like biopsies, for a temporary infection that was never going to persist or turn into cancer anyway. So you would essentially be over-treating a natural biological process. You would. Well, that makes perfect sense. And it actually leads organically into how public health policy is adapting to the science. Because Malaysia released updated national guidelines in 2023,

which dictate exactly who gets which test based on these underlying biological mechanisms. Yeah, those guidelines were a major shift. And they are backed up by data, like a major 2024 study published in PLOS-1, which is a very prominent peer-reviewed scientific journal. It featured researchers from Malaysia's Ministry of Health confirming this exact strategic approach. Because the guidelines are carefully calibrated based on age and the shifting behavior of both the virus and the human immune system. So let's break down the playbook from that PLOS-1 study, so you know it applies to. For sexually active women under 30, the recommendation is a traditional PAP smear every three years. Right. For this group, we use the PAP smear to check the windows for actual damage, deliberately bypassing the noisy false alarms of temporary viral infections. We just want to know if a virus is actually causing physical harm. Then at a critical age milestone, the strategy shifts. For women age 30 to 65, the guideline recommends switching to the HPV test. Now, why age 30 specifically?

What suddenly changes in the body? It is really about accumulated exposure and viral latency. By the time a woman is in her 30s, her immune system is typically stabilized and her exposure to new strains of HDV often decreases. Okay, so the environment changes. Yeah, so if we find high risk HPV DNA in a 35-year-old, it is statistically much more likely to be a persistent infection. One that has actively evaded her immune system and is setting up that long-term residence. Exactly. It becomes a much more significant indicator of potential precancerous development. And there is a massive logistical benefit to this specific test. Oh, yeah, because if you get a negative HPV test result, it extends the waiting period for your next screening to five full years. Five years. That is a massive mental and logistical shift from the old, annual or three routines we all grew up with. It really is. Because the HPV test looks for the viral DNA, the burglar on the perimeter, a negative result means there is absolutely no threat anywhere near the house.

It's completely clear. Yeah, and it takes years for a new HPV infection to establish itself and begin mutating cells. Therefore, a negative molecular test buys you a much longer window of certified safety. You have fewer medical appointments, fewer disruptions to your life, but you maintain the exact same, if not higher level of clinical protection. It's a huge win for patients. And finally, for women over 65, the guidelines indicate that screening is usually not needed anymore, provided their prior results were consistently normal over the past decade. Because the risk profile drops significantly at that age. If the virus hasn't managed to establish a persistent presence by the time a woman is 65, the odds of a new infection taking hold and progressing to cancer in her lifetime are vanishingly small. So we have this highly specific age-based matrix. Under 30 gets the pap smear every three years. 30 to 65 gets the HPV test every five years if negative, and over 65 usually stops entirely. If we connect this to the bigger picture, this transition toward molecular HPV testing is part of a massive,

incredibly optimistic public health initiative. Oh, you mean the 2030 goal? Yes. The PLOS-1 research notes Malaysia's ambitious target, which is full national adoption of HPV testing over pap smears by 2030. That's coming up fast. It is. And this isn't just about updating clinic protocols to save time. It sits within a broader global World Health Organization plan to eliminate cervical cancer entirely as a public health concern. We are talking about rendering a major cancer functionally obsolete for future generations. It gives me chills just thinking about it. Me too. Yeah. But here's where it gets really interesting. We have spent all this time talking about the profound microscopic differences between looking for abnormal cells versus looking for viral DNA. We have. We have talked about cytology, genetic extraction, microscopes, parameters. But physically, for you, the patient sitting on the exam table both tests look and feel exactly the same. That is a crucial point for patients to understand. The physical collection method in the clinic does not change. The profound divergence in technology happens entirely after you leave the room.

So let's walk you through the reality in the room because we need to demystify the physical procedure. It helps to know what's coming. Absolutely. So a doctor or a nurse will have you lie back on an exam table with your feed and stir-ups. They will insert a small medical instrument called a speculum to open the vaginal canal. This allows them to clearly see the cervix, which is the lower part of the uterus that connects to the vagina. And they need a very clear visual because they are targeting a very specific area called the transformation zone. What's special about that zone? Well, this is where two different types of cervical cells meet. And it happens to be the exact location where HPV most easily infiltrates. It's where abnormal pre-cancerous mutations almost always begin. Gotcha. So once they have that clear view, they use a soft, specialized plastic brush to gently collect a sample of cells directly from that transformation zone. The entire procedure takes just a few minutes. Very quick. But we have to acknowledge the reality here. Having a speculum exam can be a deeply vulnerable, anxiety-inducing experience.

Oh, absolutely. It can feel uncomfortable. It is certainly physically awkward. And for many, it is a psychological hurdle. While for most women, it isn't sharply painful. It is completely valid to dread it. It is an invasive moment and acknowledging that patient discomfort is vital. But once the clinician uses that soft brush to collect the sample, the brush is placed in a small vial of liquid preservative and sent to the laboratory. And this is where the pads finally diverge based on your age. Right. So if you are under 30 and getting a pap smear, technicians take that exact vial, extract the cells, dye them on a slide, and examine them under a microscope, visually hunting for those ag normal damaged cellular structures. But if you are over 30 and getting an HPV test, technicians take that identical vial. And instead of looking at the cells, they run it through a machine that extracts the genetic material. Searching specifically for the DNA of high risk HPV strains. Exactly. So the patient experience is just a few minutes of discomfort

and then the waiting begins. And the guidelines state that results typically take one to two weeks to arrive from the laboratory. Yes. And if the result is normal, you simply breathe a sigh of relief and wait until your next scheduled screening. Which is three years for a pap or five years for an HPV test, no further action is needed. It's that simple. But knowing the science, the cellular biology and the schedule is vital. But the final piece of the puzzle is practical access. You need to know where this specialized laboratory network actually connects to your daily life in Malaysia. Fortunately in Malaysia, the infrastructure for both pap smears and HPV tests is widely accessible. Where can you go? Well, you can schedule them at government health clinics, various private hospitals, and LPPGN centers across the country. Now, for those who might not know the acronym, LPPGN stands for the National Population and Family Development Board in Malaysia. They are a massive player in this specific area of public health. They really are. They're particularly important in this landscape because they run a national

subsidy program that dramatically lowers the barrier to entry for preventative care. Which is so important. Yeah. Eligible women, age 30 to 65, can access the highly accurate HPV DNA test entirely for free through this LPPGN program. You can't really for free. Yes. Now, outside of this specific subsidy, private hospitals do charge for the molecular test. And those prices vary depending on the healthcare provider you choose. Still, having that free access is a massive public health tool. But there is a specific, incredible detail about the LPPGN program that fundamentally changes the nature of the screening itself. Oh, the self sampling. Yes. A moment ago, we talked about the speculum, the exam table, and the vulnerability of the clinical exam, which we acknowledged is a significant psychological or physical hurdle that keeps many people from getting screened at all. Which is totally understandable. And this raises an important question about how we design medical interventions for maximum participation. Yeah. How do we get more people to do it?

If the goal is to eliminate cervical cancer, we have to eliminate the barriers to screening. Which is why the LPPGN program offers free HDV DNA self sampling. Self sampling is an absolute game changer. But how is it even possible that a patient can do this themselves? That goes back to the underlying technology of the HPV test. Okay. Remind me. Remember, a pap smear requires a doctor to aim a brush at the exact right spot the transformation zone. Yeah. To harvest perfectly intact cells for a microscopic visual inspection. Right. Checking the windows. But the HPV test isn't looking for intact cells. It is just looking for microscopic traces of viral DNA. And that viral DNA sheds naturally into the vaginal canal. Meaning the sample doesn't have to be perfectly scraped from the cervix by a professional. Nope. A patient can simply be handed a kit, go into a private bathroom, and use a swab, which is very similar to a long cotton swab, to collect a sample from the lower vagina themselves. It is really that easy. It removes the speculum from the equation entirely for those who are hesitant about the traditional clinical procedure.

It bridges the gap between sophisticated molecular laboratory science and basic human comfort. I love that. It ensures that the perimeter camera can be installed without requiring a disruptive construction crew inside the house to return to your earlier analogy. That is brilliant. And the accuracy. The accuracy of a self-collected vaginal swab for finding HPV DNA is remarkably comparable to a clinician-collected cervical sample. So, what does this all mean? It means that cervical cancer screening is not simply about early detection anymore. It is not just finding a problem while it is small so we can cut it out. No, it's moved away past that. It is literally one of the most effective ways in modern medicine to prevent a major cancer from happening in the first place. We are catching the invisible virus before the cells even begin to change or we are catching the cells before they ever turn malignant.
