Associate Professor Carlo Caponecchia and a Student Analysis of How Systems Can Fail

A/Prof Carlo Caponecchia is an academic at The University of New South Wales (UNSW) Sydney,  He has a background in psychology and works in Human Factors and Safety across several domains, with particular expertise in psychosocial risks and safe systems of work.

This podcast Carlo’s talks with some of his 3rd year Aviation students from the course  “Human Factors Tools and Methods” . During the workshop based course, students get hands on experience using a range of HFE tools, from task analysis and the Quick Exposure Check, to FRAM and STAMP.

In this podcast Carlo's students present their analysis of how systems can fail using AcciMap, and discuss what they learned about the incident, the tools and HFE as a discipline.
