Welcome to the Jack Asia Podcast. I'm Jiang Wang, editor-in-chief of Jack Asia, and the Chair of the Heart Center and the Second Affiliated Hospital of the Zhaojiang University School of Medicine in Hangzhou, China. It is my pleasure to join you today to share a brief overview of our March 2026 issue, which features six original research articles and two state-of-the-art reviews. Let's begin. Paper 1. So Tetracept in Japanese PAH Pishings. The first article in Tetra, Phase III, Open Label, March Center Study of So Tetracept in Japanese participants, with a pulmonary arterial hypertension, reports the first dedicated trial of this novel

active in signatory inhibitor in an aging population. Among 46 Japanese PAH Pishings, receiving stable background therapy, so Tetracept reduced pulmonary bacterial resistance by 99.9 second per centimeter to the fifth power and improved six-minute work distance by 42 meters at 24 weeks. Importantly, the safety profile was consistent with the prior global trials and no patients discontinued treatment due to adverse events. These findings confirmed the efficacy and the safety of the so Tetracept in Japanese PAH Pishings, and supported its potential as an important therapeutic option for this historically and third population.

The second article, Predators of Bleeding Complications, after extra corporeal cardiac pulmonary resuscitation, insights from the S-A-V-E-J-2 study analyzed 1,935 ECPR Pishings from the S-A-J-2 Registry, while adjusted bleeding rates varied by cardiac arrest ideology, highest in endogenous cardiac causes at 25.9 percent. Multi-variable analyses revealed no significant difference after adjustment, inch-a-artic pulmonary pumping emerged as the strongest independent predator,

increasing bleeding risk by 72 percent, yet paradoxically reducing mortality by 45 percent. These findings ensue the critical trade-offs in mechanical secretory support and highlight the need for vigilant hemorrhage monitoring, particularly with attractive devices. PIP-3 antithrombotic efficacy by river-roxy band adherence. The third article, medication adherence and antithrombotic therapy, outcomes, insights from a post-hug analysis of the I-FIR trial presents a post-hug analysis of the A-5 trial, examining how adherence to river-roxy band influences treatment effects, impingings with atrial-fibrillation and stable coronary artery disease. Among 200 and 120 Pishings,

94.9 percent demonstrated high adherence. In adherence, river-roxy band monotherapy showed superior efficacy and safety combined with combination therapy. Notably, these benefits disappeared in non-adherent Pishings. In high risk Pishings with angina history, adherence was associated with significantly better outcomes. These findings ensure that optimal therapeutic benefits of DOEC monotherapy depend critically on Pishing adherence. The fourth paper is titled, suitability for transcrowded, transcathed, aortic valve replacement in the Japanese population. Analyzing 336 consecutive cases, 91 percent of the Pishings were anatomically suitable

for transcrowded, kiver, including 86 percent of those with challenging transfemoral anatomy, a group that experienced substantially higher muscular complication fluids. Peripheral artery disease emerged as the sole independent predator of unsuitability. These findings established transcrowded exercise as a viable alternative in aging populations with some more body size, offering a safe option when femoral roots are compromised. Paper 5. Hematesis after-tiver using seaping through orchard resilient. The fifth study is titled, Hematesis after-tiver using the fifth generation blue and expandable valve,

comparison with the third generation valve, despite lower pervervalveular recatitation fluids, the fifth generation valve should significantly higher Hematesis incidence, 8.5 percent versus 0.9 percent. Key predators included several severe annular calcification, chronic hemodialysis, 20-minute valve sites, absence of bad block therapy, and the trees or mild pervervalveular leak. These findings highlight the multifactorial nature of this complication, and underscore the need for careful procedure optimization, and post-operative monitoring when using this widely adopted valve platform. Paper 6. AR-DH2 and diabetes in AF recurrence. The sixth article,

diabetes and AR-DH2 gene polymorphism on post-operative nature-fibrillating recurrence, prospectively followed 124 patients of their first catheditation. Investigators found that type 2 diabetes alone significantly increased recurrence rate, but the combination of diabetes with AR-DH2 variant genotype conferred the highest risk, 55.8 percent versus 16.3 percent, in those with neither factor. Multi-variable analysis confirmed this genetic metabolic interaction as the strongest independent predictor, with a four-fourth increase hazard. These findings highlight the value of integrated risk profiling for personalized post-obligine management, particularly in East Asian populations

where this variant is prevalent. Paper 7. State of the after-review of global cardiac metabolic shift. The first review, Asian, at the epicenter of the global cardiac metabolic shift. It means the escalating burden of cardiac metabolic disease across Asian. The authors highlight that metabolic risk factors not accounted for over one-third of cardiovascular mortality in the region, driven by biological susceptibility at lower BMI thresholds, rapid urbanization, and obese genic-fold environments. The emphasized critical health system gaps, including hypertension, control rates below 20 percent, in several countries, and limited access to essential therapies.

The review calls for reaching specific risk prediction tools, suggestions, primary care, and equitable access to emerging therapeutics, including SGR-2 inhibitors, and GLP-1 receptor angonists, to address these glowing crises. Paper 8. State of the after-review of cholesterol inflammation fusion. The second review is entitled cholesterol inflammation fusion hypothesis in SGR-2 and involving peridim in psogenesis and therapy. This review proposes a peridim shift in understanding SGR-2 as a self-penetrating disease, driven by bi-directional interprey between this regulated cholesterol metabolism and chronic vascular inflammation. The authors synthesize mechanistic evidence showing cholesterol,

crystals, and oxidized air-dr-activated inflammatory signaling. While cytokines impair cholesterol efflux and amplify lipid accumulation, clinical trial data consistently demonstrates that controlling either pathway along leaves substantial residual cardiovascular risk, whereas simultaneous targeting yields maximum benefit. The framework defines residual risk as a fusion risk, and calls for integrated biomarker strategies and dual pathway therapeutics to disrupt this pathogenic loop.
