Hackney Council's Social Care Services Rated 'Requires Improvement'

The report also criticized issues with continuity of care, safeguarding and smooth transitions, especially for young people shifting from children's to adult services. Some didn't get assessments until they turned 18, leaving gaps in their support plans. Council leaders acknowledged the problems openly. The cabinet member for Health and Adult Social Care called the rating fair and admitted they had it manage assessment times as well as needed. He expressed disappointment over missing the good mark, but stressed the service is heading in the right direction now.
